Pain in the outer back portion of the hip may stem from several underlying causes.1 This area of pain has traditionally been referred to as “Piriformis Syndrome”. However, more recently, many medical professionals are referring to this condition as “Deep Gluteal Pain Syndrome” which is a newly coined term. This term is an umbrella term for sciatic nerve compression withing the deep gluteal space. 2

Since it is not exclusively the piriformis muscle that can cause entrapment of the sciatic nerve, this term is thought to be more appropriate.2 Other muscles that can be involved include the gluteal muscles, and other hip external rotators. There can also be compression by blood vessels and the presence of fibrous bands formed that can cause entrapment/compression of the sciatic nerve as it traverses through the deep gluteal space.

Frequently, imbalances in strength and motor control of our deep gluteal region musculature can contribute to excessive shear forces on the sciatic nerve, leading to pain in the deep gluteal region.

Strengthening and improving motor control in these muscle groups can help reduce symptoms at the gluteal region and/or down the leg.
